Over the last several years, as cesarean deliveries have grown increasingly common, there has been a great deal of public and professional interest in the phenomenon of women 'choosing' to deliver by cesarean section in the absence of any specific medical indication. The issue has sparked intense conversation, as it raises questions about the nature of autonomy in birth. Whereas mainstream bioethical discourse is used to associating autonomy with having a large array of choices, this conception of autonomy does not (...) seem adequate to capture concerns and intuitions that have a strong grip outside this discourse. An empirical and conceptual exploration of how delivery decisions ought to be negotiated must be guided by a rich understanding of women's agency and its placement within a complicated set of cultural meanings and pressures surrounding birth. It is too early to be 'for' or 'against' women's access to cesarean delivery in the absence of traditional medical indications – and indeed, a simple pro- or con- position is never going to do justice to the subtlety of the issue. The right question is not whether women ought to be allowed to choose their delivery approach but, rather, taking the value of women's autonomy in decision-making around birth as a given, what sorts of guidelines, practices, and social conditions will best promote and protect women's full inclusion in a safe and positive birth process. This history of philosophy is a history of men. Or at least, that’s how it has been told over the past several hundred years. But, over the last few decades, we’ve begun to see more and more recognition of women philosophers and the huge impact that they have had on the course of our discipline. There have always been philosophers who happened to be women. Hypatia of Alexandria was known by her contemporaries simply as The Philosopher, and hundreds of young (...) men travelled from throughout the region to attend her public lectures. Philosophers who happen to be women, then, are nothing new. But our failure to recognise them as full contributors to the subject makes them appear to us as something of a surprise. A result of this is that women are often remembered as women first: they are seen more as women than they’re seen as philosophers. (shrink)

While risk of harm is an important focus for whether clinical research on humans can and should proceed, there is uncertainty about what constitutes harm to a trial participant. In Phase I trials on healthy volunteers, the purpose of the research is to document and measure safety concerns associated with investigational drugs, and participants are financially compensated for their enrollment in these studies. In this article, we investigate how characterizations of harm are narrated by healthy volunteers in the context of (...) the adverse events they experience during clinical trials. Drawing upon qualitative research, we find that participants largely minimize, deny, or re-attribute the cause of these AEs. We illustrate how participants' interpretations of AEs may be shaped both by the clinical trial environment and their economic motivation to participate. While these narratives are emblematic of the larger ambiguity surrounding harm in the context of clinical trial participation, we argue that these interpretations also problematically maintain the narrative of the safety of clinical trials, the ethics of testing investigational drugs on healthy people, and the rigor of data collected in the specter of such ambiguity. Most academics in philosophy departments accept that the discipline needs to change for the better. In this piece, Rebecca Buxton and Lisa Whiting offer several proposals for how to make philosophy a more inclusive place for everyone.

It is clear that philosophy has a “woman problem”. Despite the recent acceptance of this fact, it is less clear what ought to be done about it. In this paper, we argue that philosophy as a discipline is uniquely well-positioned to think through the marginalisation suffered by women and other minorities. We therefore interrogate two values that already undergird conversations about inclusion— representation and intersectionality—in order to think about the path ahead. We argue that, once we have done so, it (...) becomes clear that the slow pace of improvement over the last few decades is unacceptable and more radical steps need to be taken. First, we outline the current state of women in philosophy focusing on three areas: levels of employment, publishing, and sexual harassment. Then we turn to representation and intersectionality respectively. We conclude by arguing that many women and people of colour have been arguing for a more radically diverse philosophy for many years. Different types of consent are used to obtain human biospecimens for future research. This variation has resulted in confusion regarding what research is permitted, inadvertent constraints on future research, and research proceeding without consent. The National Institutes of Health Clinical Center's Department of Bioethics held a workshop to consider the ethical acceptability of addressing these concerns by using broad consent for future research on stored biospecimens. Multiple bioethics scholars, who have written on these issues, discussed the reasons for consent, the (...) range of consent strategies, and gaps in our understanding, and concluded with a proposal for broad initial consent coupled with oversight and, when feasible, ongoing provision of information to donors. This article describes areas of agreement and areas that need more research and dialogue. BackgroundClinical trials should be as inclusive as possible to facilitate equitable access to research and better reflect the population towards which any intervention is aimed. Informed by the UK’s National Institute for Health and Care Research (NIHR) Innovations in Clinical Trial Design and Delivery for the Under-served (INCLUDE) guidance, we audited oncology trials conducted by the Clinical Trials and Statistics Unit at The Institute of Cancer Research, London (ICR-CTSU) to identify whether essential documents were overtly excluding any groups and whether (...) sufficient data were collected to assess diversity of trial participants from groups suggested by INCLUDE as under-served by research in the UK.MethodsThirty cancer clinical trials managed by ICR-CTSU and approved between 2011–2021 were audited. The first ethics approved version of each trial’s protocol, patient information sheet, and patient completed questionnaire, together with the first case report forms (CRFs) version were reviewed. A range of items aligned with the INCLUDE under-served groups were assessed, including age, sex and gender, socio-economic and health factors. The scope did not cover trial processes in participating hospitals.ResultsData relating to participants’ age, ethnic group and health status were well collected and no upper age limit was specified in any trials’ eligibility criteria. 23/30 (77%) information sheets used at least one gendered term to address patients. Most CRFs did not specify whether they were collecting sex or gender and only included male or female categories. The median reading age for information sheets was 15–16 years (IQR: 14–15 – 16–17). Socio-economic factors were not routinely collected and not commonly mentioned in trial protocols.ConclusionsNo systemic issues were identified in protocols which would explicitly prevent any under-served group from participating. Areas for improvement include reducing use of gendered words and improving readability of patient information. The challenge of fully assessing adequate inclusion of under-served populations remains, as socio-economic factors are not routinely collected because they fall beyond the data generally required for protocol-specified trial endpoint assessments. This audit has highlighted the need to agree and standardise demographic data collection to permit adequate monitoring of the under-served groups identified by the NIHR. (shrink)

Food provisioning refers to the mental, physical and emotional labor involved in providing food for oneself and one’s family. The labor of food provisioning has been found to be made more difficult by a number of factors, including gender, socioeconomic status, age, and geography. However, little research has been done examining the labor of food provisioning among farm workers, a significantly marginalized population in the United States. In order to examine the food provisioning strategies and struggles of farm workers, we (...) have been conducting pilot research with Latinx agricultural workers in southwestern Idaho. Employing a range of qualitative and quantitative methods, we examine the various strategies that farm workers use to provide food for themselves and their families, including accessing food banks, utilizing food stamps, purchasing groceries and accepting food from friends, family and employers. We consider this from a wider lens of “caring labor”. In addition, we explore food provisioning strategies in the context of economic and demographic changes in Idaho. We observe the ways in which changing crop patterns, in particular the growth the hops industry, relates to a transition in the labor force, especially an increase in women farm workers. We have also observed a shift away from employing migrant farm labor and towards the employment of “settled” Latinx residents from surrounding communities. These changes all influence the labor of food provisioning for rural Idaho Latinx families. (shrink)

Reasoning well about risk is most challenging when a woman is pregnant, for patient and doctor alike. During pregnancy, we tend to note the risks of medical interventions without adequately noting those of failing to intervene, yet when it's time to give birth, interventions are seldom questioned, even when they don't work. Meanwhile, outside the clinic, advice given to pregnant women on how to stay healthy in everyday life can seem capricious and overly cautious. Population-level biomedical research offers new opportunities to improve population health, but also raises new challenges to traditional systems of research governance and ethical oversight. Partly in response to these challenges, various models of public involvement in research are being introduced. Yet, the ways in which public involvement should meet governance challenges are not well understood. We conducted a qualitative study with 36 experts and stakeholders using the World Café method to identify key governance challenges and explore how public involvement can (...) meet these challenges. This brief report discusses four cross-cutting themes from the study: the need to move beyond individual consent; issues in benefit and data sharing; the challenge of delineating and understanding publics; and the goal of clarifying justifications for public involvement. The report aims to provide a starting point for making sense of the relationship between public involvement and the governance of population-level biomedical research, showing connections, potential solutions and issues arising at their intersection. We suggest that, in population-level biomedical research, there is a pressing need for a shift away from conventional governance frameworks focused on the individual and towards a focus on collectives, as well as to foreground ethical issues around social justice and develop ways to address cultural diversity, value pluralism and competing stakeholder interests. There are many unresolved questions around how this shift could be realised, but these unresolved questions should form the basis for developing justificatory accounts and frameworks for suitable collective models of public involvement in population-level biomedical research governance. All rights reserved.We present new observations of the low-mass companion to HD 984 taken with the Gemini Planet Imager as a part of the GPI Exoplanet Survey campaign. Images of HD 984 B were obtained in the J and H bands. Combined with archival epochs from 2012 and 2014, we fit the first orbit to the companion to find an 18 au orbit with a 68% confidence interval between 14 and 28 au, an eccentricity (...) of 0.18 with a 68% confidence interval between 0.05 and 0.47, and an inclination of 119°with a 68% confidence interval between 114°and 125°. To address the considerable spectral covariance in both spectra, we present a method of splitting the spectra into low and high frequencies to analyze the spectral structure at different spatial frequencies with the proper spectral noise correlation. Using the split spectra, we compare them to known spectral types using field brown dwarf and low-mass star spectra and find a best-fit match of a field gravity M6.5 ±1.5 spectral type with a corresponding temperature of K. Photometry of the companion yields a luminosity of log=2.88 ± 0.07 dex with DUSTY models. Mass estimates, again from DUSTY models, find an age-dependent mass of 34 ±1 to 95 ±4 M Jup. These results are consistent with previous measurements of the object. (shrink)

The concept of autonomy plays a central role in bioethics,1 but there is no consensus as to how we should understand it beyond a general notion of self-determination. The conception of autonomy deployed in applied ethics2 can have crucial ramifications when it is applied in real-world scenarios, so it is important to be clear. However, this clarity is often lacking when autonomy is discussed in the bioethics literature. The increasing incidence of ‘trafficking’ has added an incontestably disturbing dimension to the contestable nature of a ‘non-trafficked’ UK sex industry. Men who buy sex remain under-researched, though some studies have indicated ambivalence within men's attitudes. This study combines a critical discursive psychology in support of dialogical self theory. Secondary data, from prominent UK media resources, were analysed using Edley's method of combining ‘interpretative repertoires’, ‘ideological dilemmas’ and ‘subject positions’. Contrasting discursive practices indicative of wider ideological conflict were found. Discursive (...) concepts were ‘mapped’ onto Hermans and Hermans-Konopka's ‘I-positions’ to explore how these potentially dilemmatic positions might be understood in terms of identity production. The function of ‘uncertainty’, particularly salient with the increasing complexity of globalisation, was considered a factor in how men's identities might be limited by the current discursive space. A ‘dialogical’ model of self is introduced as a framework for understanding how men who buy sex might take up new, inclusive, positions. A corollary is reflected upon; that researchers, activists and buyers alike, who remain ideologically inflexible, may be sustaining the conditions for coercion through their contribution to the discursive conflict. The synthesis of discursive and dialogical analytical tools is recommended for investigating the production of selves in contested spaces. (shrink)

There is remarkable variety in emotional life. Not all mental states referred to by the same word (e.g., “fear”) look alike, feel alike, or have the same neurophysiological signature. Variability has been observed within individuals over time, across individuals from the same culture, and of course across cultures. In this paper, I outline an approach to understanding the richness and diversity of emotional life. This model, called the conceptual act model, is not only well suited to explaining individual differences in (...) the frequency and quality of emotion, but it also suggests the counter-intuitive view that the variety in emotional life extends past the boundaries of events that are conventionally called “emotion” to other classes of psychological events that people call by different names, such as “cognitions”. As a result, the conceptual act model is a unifying account of the broad variety of mental states that constitute the human mind. (shrink)

Is it ethical to deceive the individuals who participate in psychological experiments for methodological reasons? We argue against an absolute ban on the use of deception in psychological research. The potential benefits of many psychological experiments involving deception consist in allowing individuals and society to gain morally significant self-knowledge that they could not otherwise gain. Research participants gain individual self-knowledge which can help them improve their autonomous decision-making. The community gains collective self-knowledge that, once shared, can play a role in (...) shaping education, informing policies and in general creating a more efficient and just society. (shrink)

This paper contributes to the debate about ‘tenseless languages’ by defending a tensed analysis of a superficially tenseless language. The language investigated is St’át’imcets (Lillooet Salish). I argue that although St’át’imcets lacks overt tense morphology, every finite clause in the language possesses a phonologically covert tense morpheme; this tense morpheme restricts the reference time to being non-future. Future interpretations, as well as ‘past future’ would-readings, are obtained by the combination of covert tense with an operator analogous to Abusch’s (1985) WOLL. (...) I offer St’át’imcets-internal evidence (of a kind not previously adduced) that the WOLL-like operator is modal in nature. It follows from the analysis presented here that there are only two (probably related) differences between St’át’imcets and English in the area of tense. The first is that St’át’imcets lacks tense morphemes which are pronounced. The second is that the St’át’imcets tense morpheme is semantically underspecified compared to English ones. In each of these respects, the St’át’imcets tense morpheme displays similar properties to pronouns, which may be covert and which may fail to distinguish person, number or gender. Along the way, I point out several striking and subtle similarities in the interpretive possibilities of St’át’imcets and English. I suggest that these similarities may reveal non-accidental properties of tense systems in natural language. I conclude with discussion of the implications of the analysis for cross-linguistic variation, learnability and the possible existence of tenseless languages. (shrink)

Phase 1 healthy volunteer clinical trials—which financially compensate subjects in tests of drug toxicity levels and side effects—appear to place pressure on each joint of the moral framework justifying research. In this article, we review concerns about phase 1 trials as they have been framed in the bioethics literature, including undue inducement and coercion, unjust exploitation, and worries about compromised data validity. We then revisit these concerns in light of the lived experiences of serial participants who are income-dependent on phase (...) 1 trials. We show how participant experiences shift attention from discrete exchanges, behaviors, and events in the research enterprise to the ongoing and dynamic patterns of serial participation in which individual decision-making is embedded in collective social and economic conditions and shaped by institutional policies. We argue in particular for the ethical significance of structurally diminished voluntariness, routine powerlessness in setting the terms of exchange, and incentive structures that may promote pharmaceutical interests but encourage phase 1 healthy volunteers to skirt important rules. A core area of phonology is the study of phonotactics, or how sounds are linearly combined. Recent cross‐linguistic analyses have shown that the phonology determines not only phonotactics but also the articulatory coordination or timing of adjacent sounds. In this article, I explore how the relation between coordination and phonotactics affects speakers producing nonnative sequences. Recent experimental results (Davidson 2005, 2006) have shown that English speakers often repair unattested word‐initial sequences (e.g., /zg/, /vz/) by producing the consonants with a less (...) overlapping coordination. A theoretical account of the experimental results employs Gafos's (2002) constraint‐based grammar of coordination. In addition to Gafos's Alignment constraints establishing temporal relations between consonants, a family of Release constraints is proposed to encode phonotactic restrictions. The interaction of Alignment and Release constraints accounts for why speakers produce nonnative sequences by failing to adequately overlap the articulation of the consonants. The optimality theoretic analysis also incorporates floating constraints to explain why speakers are not equally accurate on all unattested clusters. (shrink)

Irigaray's critique of the phallocentric subject's implicit dependence on the maternal-feminine “outside” is compelling. Her postulation of nonhierarchical sexual difference gives the relational world of woman specificity and Irigaray brings the subject's worldview to earth as merely the relation of the male human to the world. But the focus of her transvaluation remains largely anthropocentric; and she maintains too many aspects of the privilege of the subject's sovereignty as proper to male subjectivity. I suggest that, we need to extend Irigaray (...) and to think sexual difference beyond the human. Drawing from Elizabeth Grosz, Gilles Deleuze, Henri Bergson and Jakob Von Uexküll, I argue that we need to think life beyond the constitution of the organism as the continuous enmeshment of different milieus. Sexual difference is a force of differentiation that articulates relationships between organisms and organs as crucial aspects of many milieus that make up the Earth. Along with the sexually differentiated aspects of milieus, there are rhythms of difference constitutive of milieus that are not sexed. The first section of the essay describes Irigaray's conceptualization of sexual difference between humans. Then I turn to her engagement with the status of nonhuman animals in “Animal Compassion.” I suggest her approach in this essay is too anthropocentric and move to a discussion of Uexküll's thinking on milieus as a way to extend the concepts of difference and sexual difference into posthuman contexts. The essay concludes by reading Irigaray's concept of difference with Deleuze and Bergson. (shrink)

The pervasive effects of early childhood experiences on health at older ages, documented with methods from life course epidemiology, have served to refocus many public health efforts towards understanding the impact of both cumulative disadvantage and what are known as "sensitive periods" and "critical periods" in shaping health trajectories.
